During EECP/ECP, the patient’s lower extremities are wrapped in three compressive pneumatic cuffs applied to the calves, lower thighs, and upper thighs/buttocks. ECG-gated sequential leg compression occurs as these cuffs are inflated from distal to proximal in early diastole, creating a retrograde arterial counterpulse. Oxygenated blood is pushed toward the heart and into the coronary arteries resulting in an increase in proximal diastolic aortic pressure (diastolic augmentation), much like with the intra-aortic balloon pump. The cuffs then rapidly deflate just before the onset of systole, significantly reducing the heart’s workload by lowering peripheral vascular resistance. Unlike the intra-aortic balloon pump, the compression of the lower extremity vascular beds during EECP/ECP also increases venous return, further improving the cardiac output.
